No, most fetishes are benign and part of a fluid and evolving sexual identity. Homosexuality and heterosexuality are both two sides of the same coin likely an effect of society on puberty. I am saying that it is not a disease, thinking of mental illness as a disease is problematic because of the manner in which we perceive disease.
